What determines which prostate cancer treatment options are considered?
Prostate cancer treatment options depend mainly on where the cancer is, how it is expected to behave and your overall health.
There is no single treatment pathway that fits everyone.
A specialist may consider several factors together, including:
- whether the cancer is localized, locally advanced or metastatic
- PSA level
- Gleason score and Grade Group
- findings from imaging and biopsy
- whether the cancer remains sensitive to hormone therapy
- genetic or molecular features
- age, general health and other medical conditions
- previous prostate cancer treatments
- possible side effects
- your own priorities and preferences

The purpose of these factors is not simply to identify the most aggressive treatment available. They help a specialist discuss whether treatment is needed, what its goal would be and which trade-offs may matter in your situation.
What are the main treatment options for localized prostate cancer?
Localized prostate cancer may be managed with active surveillance, surgery, radiation therapy or, in selected circumstances, other approaches.
Which options are discussed depends heavily on the cancer’s risk group and your general health.
Active surveillance
Active surveillance involves following the cancer closely rather than starting treatment immediately.
It is an established approach for many low-risk prostate cancers and can also be considered in selected cases with more favourable intermediate-risk features.
Follow-up can involve PSA testing, clinical reviews, MRI and repeat biopsies. The purpose is to identify meaningful changes that could lead to a discussion about active treatment.
Active surveillance is different from watchful waiting. Watchful waiting generally involves less intensive follow-up and focuses on managing symptoms if they develop, often when curative treatment would be unlikely to provide meaningful benefit because of age, health or life expectancy.
Radical prostatectomy
Radical prostatectomy is surgery to remove the prostate and usually the seminal vesicles.
It can be considered for localized or some locally advanced prostate cancers when treatment with curative intent is appropriate.
Possible longer-term effects include urinary incontinence and erectile dysfunction. The likelihood and severity vary considerably between individuals.
Radiation therapy
Radiation therapy uses high-energy radiation to damage cancer cells.
Depending on the situation, this may include external beam radiation therapy or brachytherapy, in which a radiation source is placed within or close to the prostate.
Hormone therapy may also be given alongside radiation for some intermediate- and high-risk cancers.
Radiation can affect urinary, bowel and sexual function. Some effects occur during treatment, while others may appear later.

How does treatment change when prostate cancer has spread?
Metastatic prostate cancer usually requires treatment that works throughout the body rather than treatment directed only at the prostate.
A key distinction is whether the cancer is hormone-sensitive or castration-resistant.
Hormone-sensitive prostate cancer still responds to treatments that reduce or block androgen activity. Castration-resistant prostate cancer continues to progress despite testosterone being reduced to very low levels.
These terms describe how the cancer behaves and are separate from whether it has spread.
What is androgen deprivation therapy?
Androgen deprivation therapy, or ADT, reduces the effect of testosterone on prostate cancer.
For metastatic hormone-sensitive prostate cancer, current European guidelines generally recommend combining ADT with another systemic treatment when someone is fit for combination therapy, rather than relying on ADT alone.
Depending on the individual situation, combinations may include medicines such as:
- abiraterone with prednisone or prednisolone
- apalutamide
- enzalutamide
- darolutamide
- chemotherapy with docetaxel as part of selected combination regimens
The appropriate combination depends on health, disease characteristics, possible side effects and medicines already being taken.
For some people with newly diagnosed metastatic disease and a relatively low volume of cancer, radiation therapy directed at the prostate may also be discussed.
Surgery to remove the prostate is not a standard treatment for newly diagnosed metastatic disease outside clinical research.
What happens if prostate cancer becomes castration-resistant?
Castration-resistant prostate cancer can still have several treatment options, even though it is progressing despite very low testosterone levels.
Treatment becomes increasingly individualized at this stage.
Depending on where the cancer has spread, previous medicines, symptoms and molecular characteristics, options can include:
- androgen-receptor pathway inhibitors
- chemotherapy such as docetaxel or cabazitaxel
- PARP inhibitors for selected cancers with relevant DNA-repair gene alterations
- radioligand therapy such as lutetium-177 PSMA therapy for selected PSMA-positive disease
- radium-223 in certain situations involving bone metastases
- other biomarker-directed treatments in specific cases
- clinical trials
The sequence matters. A medicine that is useful at one stage may offer less benefit after a closely related treatment has already stopped working.

When can genetic testing affect prostate cancer treatment options?
Genetic and tumour testing can reveal changes that make particular treatments relevant for some forms of advanced prostate cancer.
Genes involved in repairing damaged DNA are especially important. These include BRCA1, BRCA2 and other homologous recombination repair, or HRR, genes.
Certain alterations can affect whether a PARP inhibitor is an option.
Testing can involve tumour tissue, blood or saliva depending on whether a specialist is looking for changes found only in the cancer or inherited changes that may also have implications for biological relatives.
Genetic testing does not automatically mean a particular treatment will be suitable. The result needs to be interpreted alongside disease stage, previous therapy and other clinical factors.
What side effects can matter when comparing prostate cancer treatments?
The side effects that matter most can differ substantially between surgery, radiation therapy and systemic treatments.
This makes treatment decisions partly about trade-offs.
Possible effects include:
After surgery: urinary leakage, erectile dysfunction and changes in ejaculation.
After radiation therapy: urinary irritation, bowel changes, fatigue and sexual difficulties. Some effects can develop months or years later.
During ADT: hot flushes, reduced sexual desire, erectile difficulties, fatigue, loss of muscle mass, changes in body composition, bone loss and metabolic changes.
During chemotherapy: fatigue, reduced blood counts, infection risk, hair loss, numbness or tingling and gastrointestinal effects can occur, depending on the medicine.
Other systemic therapies have their own side-effect profiles.
Not everyone experiences the same effects or experiences them to the same degree. Asking about both short- and long-term effects can help you understand how different choices could affect everyday life.
What do experiences shared through the mama health app reveal about treatment decisions?
Information shared through the mama health app shows that understanding treatment can be almost as important as learning the name of the treatment itself.
Across structured information shared by men in Italy living with prostate cancer, surgery, active surveillance and ADT frequently appeared among the first approaches described.
The information also highlights several recurring concerns.
Side effects stood out most clearly. Around 21% of the information reviewed included treatment-related side effects as an important difficulty. Concerns ranged from urinary and sexual changes to fatigue and the wider effects of hormone therapy.
Understanding the disease was another recurring challenge. Around 14% reflected gaps in disease education. Knowing the treatment name was not always the same as understanding why it had been chosen or what might happen next.
Uncertainty about whether treatment was working also appeared repeatedly. Around 9% included concerns about treatment effectiveness.
Specialists were the information source mentioned most often during treatment planning. Among those who described how useful the information they received had been, 62% rated it as very useful.
These patterns cannot determine which treatment is appropriate for one individual. They can, however, highlight questions that others have found important when preparing for discussions with their healthcare team.
How can you compare prostate cancer treatment options?
A useful comparison starts with the goal of treatment, followed by expected benefits, possible side effects and what each choice would mean for daily life.
Instead of asking only “Which treatment is best?”, it can help to break the decision into smaller questions.
You could ask:
- What is my exact stage, Grade Group and risk category?
- What is the goal of this option in my situation?
- Which other reasonable options could be considered?
- What are the most likely short-term and long-term side effects?
- How could this treatment affect urinary, bowel and sexual function?
- If I choose active surveillance, what changes could lead to a discussion about treatment?
- If I start this treatment, what other options might still be available later?
- Could genetic or tumour testing provide useful additional information in my case?
You do not need to remember every treatment name before an appointment. Knowing your stage and understanding the reason behind the options being discussed can give the conversation a clearer structure.

Is there always one best prostate cancer treatment?
No. Several prostate cancer treatments can be medically reasonable in the same situation, particularly when cancer is still localized.
For example, someone with a lower-risk localized cancer may be able to consider active surveillance, while someone with disease that requires local treatment may have both surgery and radiation among the options discussed.
The outcomes that matter can also differ from person to person.
One person may place greater importance on avoiding surgery. Another may be particularly concerned about bowel effects from radiation. Someone else may want to understand sexual or urinary effects before comparing options.
In advanced disease, choices can depend strongly on previous treatment, disease distribution, molecular findings and overall health.
Shared decision-making therefore involves both medical evidence and what matters to you.
What is the most important thing to understand about prostate cancer treatment?
The most important point is that the treatment name alone does not tell you whether it is the right option for your situation.
Stage, risk, hormone sensitivity, previous therapy, genetic features and overall health all change the treatment landscape.
