What can a colonoscopy show in Crohn’s disease?

A colonoscopy allows a doctor to look directly at the inside of your colon and usually the terminal ileum, the last part of the small intestine.

During the procedure, a flexible tube with a camera is passed through the rectum and around the colon.

The doctor looks at the bowel lining for signs of inflammation and other abnormalities.

In Crohn’s disease, this can include:

  • redness and swelling
  • ulcers
  • patchy areas of inflammation
  • changes to the normal surface of the bowel
  • narrowing
  • bleeding or fragile tissue
  • inflammation in the terminal ileum

The doctor can also take biopsies, which are tiny tissue samples examined under a microscope.

This is important because what the bowel looks like through the camera and what the tissue looks like under a microscope provide different information.

What does Crohn’s disease look like on a colonoscopy?

Crohn’s can have several characteristic appearances, but there is no single colonoscopy finding that appears in everyone with the condition.

One feature doctors may see is patchy inflammation.

Unlike inflammation that runs continuously along the bowel, Crohn’s can affect one area while leaving another area relatively normal. These separated areas are sometimes called skip lesions.

Doctors may also see ulcers.

These can range from small superficial ulcers to larger or deeper areas of ulceration.

More pronounced Crohn’s inflammation can sometimes create a cobblestone appearance, where areas of swollen tissue are separated by deeper ulcers.

Inflammation around the terminal ileum is also common in Crohn’s.

These findings can support a diagnosis, but they are not interpreted on their own. Your gastroenterologist will consider the overall pattern and other test results before deciding what the findings mean. [1,2]

What does “ileitis” mean on a colonoscopy report?

Ileitis means inflammation of the ileum, the final part of the small intestine.

The terminal ileum is particularly relevant in Crohn’s because it is a common location for the disease.

If your report says terminal ileitis, it means the doctor saw evidence of inflammation in this area.

It does not, by itself, mean you definitely have Crohn’s disease.

Ileitis can have causes other than Crohn’s, including certain infections and medication-related injury. The appearance of the inflammation, biopsy findings, symptoms, medical history, and other investigations help doctors work out the cause. [1,2]

Aphthous ulcers are small areas of ulceration. More active or severe inflammation can be associated with larger or deeper ulcers.

What does a stricture mean on your colonoscopy report?

A stricture is an area where the bowel has become narrowed.

Crohn’s can lead to narrowing because of active inflammation, longer-term scarring, or a combination of both.

During a colonoscopy, the doctor may see a narrowed area or find that the scope cannot safely pass through part of the bowel.

A stricture can be important because significant narrowing can make it harder for food and bowel contents to pass through.

But a colonoscopy cannot always tell doctors everything they need to know about a stricture.

Imaging such as magnetic resonance enterography (MRE), intestinal ultrasound, or CT enterography may provide more information about the bowel wall and areas beyond the reach of the colonoscope. [2]

If a stricture appears on your report, ask:

“How significant is the narrowing, and do I need another test to assess it?”

If you develop severe abdominal pain, persistent vomiting, a swollen abdomen, or an inability to pass stool or gas, seek urgent medical assessment because these can occur with bowel obstruction.

Why are biopsies taken during a Crohn’s colonoscopy?

Biopsies let a pathologist examine tiny samples of bowel tissue under a microscope.

This can reveal changes that are not obvious from the colonoscopy camera alone.

Biopsies may be taken from inflamed areas and from areas that appear normal.

When Crohn’s is being investigated, the pathology findings can help doctors understand the type and pattern of inflammation and distinguish between possible causes.

What does a granuloma mean on a Crohn’s biopsy?

A granuloma is a particular collection of immune cells that can sometimes be seen under the microscope in Crohn’s disease.

When an appropriate type of granuloma is found in the right clinical context, it can support a Crohn’s diagnosis.

But there are two important things to know.

First, many people with Crohn’s do not have granulomas on their biopsies.

Not finding one does not rule Crohn’s out.

Second, granulomas can have other causes, so the pathologist and gastroenterologist still interpret the finding in the context of the whole investigation.

Do not worry if your biopsy report does not mention granulomas.

They are one possible clue, not a requirement for Crohn’s disease.

Can a colonoscopy confirm a Crohn’s disease diagnosis?

A colonoscopy with biopsies is an important part of diagnosing Crohn’s disease, but there is no single test that confirms every case.

Doctors generally make a Crohn’s diagnosis using a combination of information.

That can include:

  • your symptoms and medical history
  • physical examination
  • blood tests
  • stool tests
  • ileocolonoscopy and biopsies
  • imaging of the small intestine

Current ECCO diagnostic guidance recommends ileocolonoscopy with biopsies together with intestinal imaging as first-line investigations in people with suspected inflammatory bowel disease. [2]

That is because Crohn’s can occur anywhere in the digestive tract and can affect the bowel in ways that colonoscopy alone cannot fully assess.

So you may finish a colonoscopy and still be asked to have another test.

That does not necessarily mean the colonoscopy was inconclusive or unsuccessful.

The tests answer different questions.

Can you have Crohn’s disease if your colonoscopy is normal?

Yes. A normal colonoscopy does not always exclude Crohn’s disease.

A standard colonoscopy examines the colon and usually the terminal ileum.

But Crohn’s can affect other parts of the small intestine that the scope cannot reach.

If your symptoms, blood tests, stool tests, or other information continue to raise concern about Crohn’s despite a normal colonoscopy, your healthcare team may consider additional testing.

What does a normal colonoscopy mean if you already have Crohn’s?

If you have established Crohn’s and your colonoscopy shows little or no visible inflammation, that can be encouraging.

It may indicate endoscopic remission or healing, depending on the findings and terminology used.

This matters because modern Crohn’s treatment aims beyond symptom control alone.

You may feel well while inflammation remains. Conversely, you can have some digestive symptoms even when Crohn’s inflammation has improved considerably.

What does “mild,” “moderate,” or “severe” inflammation mean?

That is not necessarily identical to describing your overall Crohn’s disease as mild, moderate, or severe.

Your overall situation can depend on much more than the appearance of one area.

  • strictures, fistulas, or abscesses
  • nutritional effects

Why might your colonoscopy results and symptoms not match?

Because symptoms and Crohn’s inflammation do not always move together.

You can have substantial symptoms without a large amount of visible inflammation.

You can also feel relatively well despite inflammation still being present.

If your colonoscopy looks reassuring but you are still having diarrhea, pain, or bloating, that does not mean your symptoms are imaginary.

Your healthcare team may consider other explanations and decide whether further assessment is needed.

Likewise, if you feel well but your colonoscopy still shows inflammation, your doctor may want to discuss whether the current treatment is achieving its intended target.

What happens after a colonoscopy suggests Crohn’s disease?

The next step depends on how complete the diagnostic picture is.

If biopsies were taken, you may need to wait for the pathology report.

Your healthcare team may also want additional blood tests, stool tests, or imaging.

Once the available information has been reviewed, your gastroenterologist can discuss whether the findings support a Crohn’s diagnosis and what they mean for you.

If Crohn’s is diagnosed, the next questions usually become:

Where is the disease?

How active is it?

Are there complications?

What treatment makes sense?

What happens after a colonoscopy if you already have Crohn’s?

If the colonoscopy was performed to assess established Crohn’s, what happens next depends on what it shows.

If inflammation has improved substantially, your healthcare team may continue the current maintenance strategy.

If significant inflammation remains, they may consider whether the current treatment is achieving its goal.

That does not mean one abnormal colonoscopy result automatically leads to a new medicine.

Your gastroenterologist may consider your symptoms, biomarkers, imaging, how long you have been taking the treatment, previous response, and other factors before deciding what to do.

What should you ask your doctor about your Crohn’s colonoscopy results?

You do not need to understand every line of the report.

A few questions can make the findings much clearer:

  • Which parts of my bowel did you examine?
  • Where did you find inflammation?
  • Was my terminal ileum affected?
  • Were there ulcers?
  • Did you see any narrowing?
  • Were you able to examine the whole colon and terminal ileum?
  • Where were biopsies taken?
  • What did the biopsies show?
  • Do these results support a Crohn’s diagnosis?
  • Do I need imaging of the small intestine?
  • If I already have Crohn’s, has the inflammation improved since my previous assessment?
  • What happens next?
  • Do these findings change my treatment?

If your doctor uses a term you do not understand, ask them to explain it.

Medical reports are written primarily to communicate between healthcare professionals. You are not failing some test by finding them difficult to read.

What blood tests are used for Crohn’s disease?

Blood tests can help doctors look for inflammation, anemia, nutritional problems, and other changes that may occur with Crohn’s disease.

If Crohn’s is being investigated or followed up, you may have several tests taken from the same blood sample.

These can include:

  • a full blood count, or CBC/FBC
  • C-reactive protein (CRP)
  • ferritin and other iron studies
  • albumin
  • electrolytes
  • liver tests
  • vitamin B12
  • folate
  • vitamin D in some circumstances

Which tests you need depends on your Crohn’s, symptoms, treatment, previous surgery, and other individual factors.

Blood tests can provide important clues.

But they do not work as a simple positive-or-negative Crohn’s test.

Can a blood test diagnose Crohn’s disease?

No single blood test can confirm that you have Crohn’s disease.

This is important if you have been told that one of your inflammatory markers is high and are now wondering whether that means you definitely have Crohn’s.

It does not.

Doctors diagnose Crohn’s using a combination of your medical history, symptoms, laboratory tests, endoscopy with biopsies, and imaging when appropriate. [1,2]

Blood tests can support that investigation.

For example, they might show inflammation or anemia.

But inflammation has many possible causes, and anemia can occur for many reasons unrelated to Crohn’s.

The blood tests provide pieces of evidence, not the diagnosis by themselves.

What does CRP mean in Crohn’s disease?

CRP, or C-reactive protein, is a blood marker that can rise when there is inflammation in the body.

You may have CRP checked when Crohn’s is first being investigated, when symptoms get worse, or while your healthcare team is assessing how you are responding to treatment.

If your CRP is elevated, it tells your healthcare team that inflammation may be present.

What it does not tell them is exactly where that inflammation is coming from.

An infection and other inflammatory conditions can also increase CRP.

That means:

High CRP ≠ definitely active Crohn’s.

There is another limitation.

Some people with active Crohn’s can have a normal CRP. Current ECCO guidance specifically notes that laboratory markers of chronic inflammation can sometimes remain normal in people with Crohn’s disease. [2]

So:

Normal CRP ≠ definitely no active Crohn’s.

That is why CRP is interpreted alongside other information rather than treated as a yes-or-no test.

Is a falling CRP a sign that Crohn’s treatment is working?

A falling CRP can be an encouraging sign if your CRP was previously raised because of Crohn’s inflammation.

For example, if your CRP was elevated before treatment and then falls alongside improving symptoms and other inflammatory markers, that can contribute to evidence that inflammation is improving.

But CRP alone cannot confirm remission.

Modern Crohn’s monitoring increasingly combines symptoms with objective measures of inflammation. Depending on your situation, those measures can include fecal calprotectin, CRP, imaging, and endoscopy. [2]

So rather than focusing only on whether one number has entered the laboratory's “normal” range, it can be more useful to ask:

“How has this changed from my previous result, and does it match what my other tests are showing?”

What stool tests are used for Crohn’s disease?

Stool tests can help doctors look for intestinal inflammation and infections.

The test you are particularly likely to encounter with Crohn’s is fecal calprotectin.

Your healthcare team may also request stool tests to look for infections when symptoms such as diarrhea suddenly develop or worsen.

That distinction matters.

A stool sample can be used to answer different questions depending on what the laboratory has been asked to test.

One sample might be looking for evidence of intestinal inflammation.

Another might be looking for bacteria or another infectious cause of your symptoms.

What is fecal calprotectin?

Fecal calprotectin is a protein measured in stool that can indicate inflammation in the intestines.

When immune cells involved in inflammation are active in the intestinal lining, calprotectin levels in stool can increase.

That makes fecal calprotectin particularly useful in inflammatory bowel disease.

Current ECCO guidance describes fecal calprotectin as an important biomarker when evaluating IBD, and it is widely used during diagnosis and follow-up. [2]

The test is useful partly because it provides information about what may be happening inside the bowel without requiring an invasive procedure every time.

That does not mean calprotectin can replace colonoscopy or imaging in every situation.

It answers a different question.

What does a high fecal calprotectin result mean?

A high fecal calprotectin result suggests that inflammation may be present in the gastrointestinal tract.

If you already have Crohn’s disease, an increase can be one clue that inflammatory activity has increased.

If you are being investigated for Crohn’s, an elevated result can support the need for further investigation.

But a high calprotectin level does not mean:

“You definitely have Crohn’s disease.”

Fecal calprotectin is a marker of intestinal inflammation, not a Crohn’s-specific marker.

Other gastrointestinal conditions, including some infections, can also increase it. [2]

Your healthcare team therefore considers the result alongside your symptoms, medical history, other tests, and sometimes colonoscopy or imaging.

What is a normal fecal calprotectin level?

This is where it is tempting to search for one number and compare your result against it.

Be careful.

Laboratories and clinical pathways can use different reference ranges and thresholds, and the significance of a result also depends on why the test was performed and your individual clinical situation.

There is not one universal calprotectin number that should be interpreted identically for every patient.

Your healthcare team may also be interested in the trend.

A result falling substantially after treatment can provide different information from a similar result that has been steadily rising over several tests.

How high is it compared with my previous result?

What does my laboratory consider the reference range?

Does this result match my symptoms and other tests?

Do I need another test?

Yes.

Similarly, you can experience digestive symptoms even when inflammatory markers are reassuring.

Fecal calprotectin can sometimes identify changes in inflammatory activity before obvious symptoms appear. [3]

That is why your gastroenterology team may continue asking for stool samples even when you feel completely fine.

They are not only asking:

“How do you feel?”

They are also asking:

“Is there evidence that the bowel is still quiet?”

Why do doctors test stool for infection when you have Crohn’s?

Because worsening diarrhea does not automatically mean you are having a Crohn’s flare.

Certain gastrointestinal infections can produce symptoms that look very similar to active inflammatory bowel disease.

Current diagnostic guidance recommends excluding gastrointestinal infection during the assessment of suspected or worsening IBD. [2]

Depending on your symptoms and circumstances, your healthcare team may test stool for infections such as Clostridioides difficile (C. diff) and other pathogens.

This is important because the treatment for an infection can be very different from the treatment for active Crohn’s inflammation.

If your symptoms suddenly worsen, do not assume that a familiar pattern automatically means “another flare.”

Your healthcare team may want to establish what is causing the change first.

What does a full blood count tell you about Crohn’s disease?

A full blood count (FBC)—also called a complete blood count (CBC)—looks at different types of cells in your blood.

Several parts can be relevant in Crohn’s.

These include:

Hemoglobin: can show whether you have anemia.

Red blood cells: carry oxygen around the body and can be affected by anemia.

White blood cells: can increase with inflammation or infection, although the result is not specific to Crohn’s.

Platelets: can sometimes rise as part of an inflammatory response.

The report may contain many additional abbreviations and numbers.

Do not assume every result marked slightly outside the laboratory reference range is caused by Crohn’s or requires treatment.

The pattern and clinical context matter.

Why can Crohn’s disease cause anemia?

Anemia is common in inflammatory bowel disease, and Crohn’s can contribute to it in several different ways. [3,4]

One important cause is iron deficiency.

Chronic intestinal blood loss can reduce iron stores. Poor appetite and dietary restriction can also affect iron intake.

Inflammation itself can contribute to another type called anemia of chronic disease, or anemia of inflammation.

Crohn’s affecting the small intestine—or previous surgery involving certain parts of it—can also contribute to deficiencies such as vitamin B12 deficiency.

This is why seeing “anemia” on a blood test does not automatically tell you what is causing it.

Your healthcare team needs to establish which type of anemia you have before deciding what it means and what should happen next.

What does low hemoglobin mean?

Hemoglobin is the protein inside red blood cells that carries oxygen around your body.

A low hemoglobin level can indicate anemia.

Symptoms of anemia can include:

  • fatigue
  • weakness
  • shortness of breath
  • dizziness
  • headaches
  • feeling unusually tired during physical activity

But symptoms alone cannot tell you whether you are anemic.

And a low hemoglobin result does not tell you the cause.

If your hemoglobin is low, your healthcare team may look at other results—including ferritin, transferrin saturation, red blood cell size, vitamin B12, and folate—to understand what is happening.

What does ferritin mean in Crohn’s disease?

Ferritin is a protein that stores iron, and measuring it can help assess your body's iron stores.

A low ferritin level can support a diagnosis of iron deficiency.

But ferritin becomes more complicated when inflammation is present.

Ferritin can increase as part of the inflammatory response. That means someone with active Crohn’s can have iron deficiency even when ferritin is not as low as you might expect.

Current ECCO guidance therefore emphasizes interpreting iron studies in the context of inflammation rather than looking at ferritin alone. [3,4]

Your healthcare team may also look at transferrin saturation, often shortened to TSAT, and CRP when assessing possible iron deficiency.

This is a good example of why individual lab results should not be interpreted in isolation.

What does MCV mean on your blood test?

MCV stands for mean corpuscular volume.

It describes the average size of your red blood cells.

Doctors can use MCV as one clue when investigating anemia.

Smaller-than-usual red blood cells can occur with iron deficiency.

Larger red blood cells can occur with vitamin B12 or folate deficiency and in some other circumstances.

Red blood cells can also remain within the normal size range in some forms of anemia, including anemia related to chronic inflammation.

MCV does not diagnose the cause by itself.

It helps your healthcare team decide which possibilities need to be investigated further.

Why might vitamin B12 be checked with Crohn’s disease?

Vitamin B12 is absorbed in the final part of the small intestine, called the terminal ileum.

That area is commonly affected by Crohn’s disease.

People with Crohn’s involving the ileum—and particularly people who have had part of the ileum surgically removed—can therefore be at greater risk of vitamin B12 deficiency.

B12 deficiency can contribute to anemia and can also cause neurological symptoms.

Your healthcare team may monitor B12 depending on where your Crohn’s is located, your surgical history, symptoms, and previous results. [3]

If you see B12 on your laboratory report, it is not necessarily because your doctor already thinks you are deficient.

It may simply be part of checking whether Crohn’s has affected your nutritional status.

Why are folate and vitamin D sometimes checked?

Crohn’s disease can make adequate nutrition more difficult for several reasons.

Inflammation, reduced appetite, dietary restrictions, malabsorption, surgery, and some medications can all affect nutritional status.

Depending on your situation, your healthcare team may therefore check nutrients such as folate or vitamin D.

Not everyone needs exactly the same nutritional blood tests at exactly the same intervals.

Your disease location, treatment, diet, symptoms, and previous results help determine what is useful.

And if a result is low, the next step is not simply to buy the highest-dose supplement you can find.

Ask your healthcare team whether supplementation is needed and what form or dose is appropriate.

What does albumin mean in Crohn’s disease?

Albumin is a protein made by the liver and measured in the blood.

A low albumin level can occur for several reasons.

In someone with Crohn’s, it may be associated with significant inflammation, poor nutritional status, protein loss through the intestine, or another health problem.

It is not a Crohn’s-specific test.

That means a low albumin result needs to be interpreted alongside other blood tests and your wider medical situation.

Similarly, a normal albumin result does not prove that Crohn’s is inactive.

Why are liver tests checked if Crohn’s affects the bowel?

Your healthcare team may check liver enzymes even though Crohn’s primarily affects the digestive tract.

There are several reasons.

Some conditions affecting the liver and bile ducts can occur alongside inflammatory bowel disease.

Certain Crohn’s medications can also affect the liver, which means blood tests may be used as part of medication safety checks.

Baseline liver testing is also included in current biochemical assessment recommendations for people diagnosed with IBD. [2]

So seeing liver tests on your blood request does not necessarily mean your doctor thinks there is something wrong with your liver.

They may simply be part of routine assessment or treatment follow-up.

Why do you need blood tests when taking Crohn’s medication?

Some Crohn’s treatments require laboratory monitoring.

Depending on the medicine, your healthcare team may check things such as:

  • blood cell counts
  • liver function
  • kidney function
  • inflammatory markers

Tests may be needed before treatment begins and periodically while you are taking it.

The exact schedule depends on the medication.

These tests can help your healthcare team assess treatment safety and provide information about your wider health.

Do not skip planned blood tests because you feel well.

Some laboratory changes do not cause obvious symptoms at first.

Can blood and stool tests tell whether Crohn’s treatment is working?

They can provide important evidence, but they are only part of the assessment.

If treatment is working, your healthcare team may see changes such as:

  • CRP decreasing if it was previously elevated
  • fecal calprotectin decreasing
  • anemia improving
  • nutritional markers improving

Those changes can be reassuring.

But current Crohn’s care does not define treatment success from one laboratory number.

Your symptoms matter too.

And depending on your disease, doctors may use imaging or endoscopy to assess whether inflammation inside the bowel has improved.

Why might your blood tests look normal when you still have symptoms?

Normal blood tests do not automatically mean there is nothing wrong.

CRP, for example, can remain normal in some people despite active Crohn’s disease. [2]

And symptoms such as diarrhea, pain, bloating, or fatigue can have causes that do not necessarily produce obvious abnormalities in routine blood tests.

Your healthcare team may therefore consider your symptoms, stool markers, previous disease pattern, and other investigations.

If symptoms have changed significantly, do not dismiss them because your blood results appear normal.

Tell your healthcare team what is happening.

Why might your tests be abnormal when you feel completely well?

The reverse can happen too.

You might feel fine while fecal calprotectin or another inflammatory marker has changed.

That does not automatically mean a major flare is about to happen.

But it can give your healthcare team a reason to look more closely.

One of the reasons Crohn’s is monitored even during remission is that objective signs of inflammatory activity can sometimes change before symptoms become obvious. [2,3]

Your healthcare team may repeat the test, compare it with previous results, or decide whether another assessment is needed.

An abnormal result is information.

It is not, by itself, a treatment decision.

What matters more: your result or the trend?

Often, both matter.

A single result is a snapshot.

A series of results can show direction.

Imagine your fecal calprotectin has been measured several times after treatment begins.

One result tells your healthcare team where the marker is today.

Several results can show whether it is generally falling, stable, or increasing.

The same principle can apply to CRP, hemoglobin, ferritin, and other laboratory results.

That does not mean you should try to create your own interpretation from a graph in your patient portal.

But it does mean one number makes more sense when you know what came before it.

A useful question at your next appointment is:

“How does this compare with my previous results?”

What happens if your Crohn’s blood or stool tests are abnormal?

There is no single next step.

It depends on which result is abnormal, how abnormal it is, whether it has changed, your symptoms, and your Crohn’s history.

Your healthcare team might:

  • repeat the test
  • request additional blood or stool tests
  • investigate a possible nutritional deficiency
  • check for infection
  • arrange imaging
  • recommend colonoscopy or another endoscopic test
  • review your current treatment

An abnormal result does not automatically mean your medication needs to change.

And it does not automatically mean your Crohn’s has become more severe.

The result needs to be understood in context first.

What should you ask about your Crohn’s lab results?

You do not need to become an expert in laboratory medicine to understand your care.

A few questions can make the conversation much clearer:

  • What was this test measuring?
  • Is this result outside the expected range?
  • How does it compare with my previous result?
  • Could something other than Crohn’s explain it?
  • Do my CRP and calprotectin tell the same story?
  • Am I anemic?
  • If I am anemic, do we know why?
  • Are my iron stores low?
  • Do I need B12, folate, or other nutritional tests?
  • Do we need to repeat any of these tests?
  • Does this result mean I need another investigation?
  • Does anything here change my treatment?

And if your results appear online before your appointment, remember:

a red number is not a diagnosis.

Reference ranges help identify results that may deserve attention. They do not provide the medical context needed to interpret what that result means for you.

What should you remember when reading Crohn’s blood and stool results?

Your results are pieces of a bigger picture.

CRP can tell your healthcare team something about inflammation in your body, but it cannot tell them by itself whether Crohn’s is active.

Fecal calprotectin can provide useful information about intestinal inflammation, but an elevated result does not automatically diagnose Crohn’s.

Hemoglobin can show anemia, but it cannot tell you why you are anemic.

Ferritin can provide information about iron stores, but inflammation can make it harder to interpret.

That is why Crohn’s care rarely comes down to one number.

Instead of asking:

“Is this result good or bad?”

try asking:

“What does this result tell us, how does it compare with my previous results, and what do we need to know next?”

That is usually where the useful information begins.
